We at Three Rivers Hospice provide care for patients and support for their families when they are facing a diagnosis of a life-limiting illness.  Our goal is to help people live each day to the fullest by focusing on comfort care, pain management, and relief from distressing symptoms.  The patient and care-giver are not alone on this journey; the Hospice Team is by their side.
Our Hospice Team:

Our Medical Directors work with the patient's physician and other team members to manage the care of the patient.
Our Registered Nurses and Licensed Practical Nurses provide hands-on care to our patients and are specially trained in end of life issues.
Our Certified Home Health Aides are available to help our patients with personal care, such as bathing, dressing and changing bed linens.
Our Social Workers assist with emotional and financial issues and help with connections to other agencies that may benefit our patients and their family members.
Our Hospice Chaplain works in conjunction with the patient's clergy to provide spiritual support for our patients and families as well as assist with funeral planning.
Our Volunteers provide companionship to our patients and respite for their caregivers - allowing them time to run errands, schedule appointments, to do housework or just rest.
Our Bereavement Counselor provides grief and loss counseling, support groups, workshops and memorial services.  Our support does not end with the loss of a loved one; we provide unlimited bereavement services to all family members as long as it is needed.
Our Pet Therapists, Anastasia, our Certified Pet Therapy Dog, and Kiwi, our Therapy Bird, work exclusively for Three Rivers Hospice, "bringing smiles to those who need them."
Other Services Provided
  • Medical equipment, supplies and medications related to the life limiting diagnosis.
  • In-Patient respite care for up to 5 days in a contracted skilled facility.
  • General in-patient hospital care at hospitals and contracted facilities when needed.
  • Continuous in-home care during a brief period of crisis.
Who Pays for Hospice?
Hospice is a 100% covered benefit under the Medicare/Medicaid programs, under the VA benefit and various private insurances.  Our Social Workers can provide specific coverage information.
