At Three Rivers Hospice and Palliative Care, our goal is to help patients live their last days of life to their fullest by focusing on their comfort, controlling their pain, and relieving any distressing symptoms 24 hours a day, 7 days a week. The patient and caregivers are not alone on this journey, the Three Rivers Hospice Team is by their side.
Our Medical Directors work with the patient's physician and other team members to manage the care of the patient.
Our Registered Nurses and Licensed Practical Nurses provide hands-on care to our patients and are specially trained in end of life issues.
Our Certified Home Health Aides are available to help our patients with personal care, such as bathing, dressing and changing bed linens.
Our Social Workers assist with emotional and financial issues and help with connections to other agencies that may benefit our patients and their family members.
Our Hospice Chaplain works in conjunction with the patient's clergy to provide spiritual support for our patients and families as well as assist with funeral planning.
Our Volunteers provide companionship to our patients and respite for their caregivers - allowing them time to run errands, schedule appointments, to do housework or just rest.
Our Bereavement Counselor provides grief and loss counseling, support groups, workshops and memorial services. Our support does not end with the loss of a loved one; we provide unlimited bereavement services to all family members as long as it is needed.
Our Pet Therapists, Anastasia and Sebastian, Certified Pet Therapy Dogs, and Kiwi, our Therapy Bird, work exclusively for Three Rivers Hospice, "bringing smiles to those who need them."
Other Services Provided
- Medical equipment, supplies and medications related to the life limiting diagnosis.
- In-Patient respite care for up to 5 days in a contracted skilled facility.
- General in-patient hospital care at hospitals and contracted facilities when needed.
- Continuous in-home care during a brief period of crisis.
Who Pays for Hospice?
Hospice is a 100% covered benefit under the Medicare, Medicaid and most private insurance programs, under the VA benefit Our Social Workers can provide specific coverage information.
